Family Child Care  Support Program Application

This form is for anyone wanting to complete the licensing process with CAFCC Foundation and/or continue their education as an established provider.

The Family Child Care Support Program includes a mentor that is a licensed family child care home provider, the mentor will guide/assist you with completing forms and applications, direct you on where/how to sign up and complete classes (Pre-Licensing, First Aid/ CPR, Business Class), give you resources and tools to help your business be successful, and complete a walkthrough of your space and give tips for a successful licensing visit.

Are you currently a licensed family child care home provider?
Have you completed your licensing application?
Do you have your zoning verification letter from your city/county?
Have you started the required training on PDIS for your license?

Class(s): You must attend & participate in all training hours to receive a certificate of completion.
